The 2009 US Open champion had been out of action for around four weeks, but eliminated Brands 6-7 (5/7), 7-6 (7/4), 7-6 (8/6) in a match featuring 59 aces and no breaks of serve.
Del Potro came through on his second match point, but only just. A Hawkeye replay was required to confirm his progress.
“If I’d known I’d have to start with a three-hour win, I would have stayed home,” he laughed.
“We both served so well, it was a really tough match.”
Del Potro still harbours hope of reaching the ATP World Tour Finals next month.
