Juve will pay €1.5m for the loan, while they will have the option of making the deal permanent for €24m next summer.
It is reported that Cuadrado will travel to Turin on Monday to undergo his medical and put pen to paper, as he looks set to leave his Premier League nightmare behind.
The Colombian international made his name with Fiorentina before his switch to Stamford Bridge, but he struggled to adapt to life in England and failed to impress Jose Mourinho.
As a result, he will likely relish the opportunity to return to Italy where he will hope he can rediscover his best form with the reigning Serie A champions.
Juve got off to a nightmare start on the opening weekend of the season, losing 1-0 at home to Udinese on Sunday.
