Rahman has impressed for German outfit Augsburg this season, creating four assists from left back as he helped his side to a surprise fifth-placed finish.
According to Daily Star, Blues boss Jose Mourinho is interested in the £20m-rated star and is understood to have made contact with Ghana boss and former Chelsea manager Avram Grant in regards to the 20-year-old.
Mourinho is believed to be looking to bolster his defence for next season after Felipe Luis failed to make an impact after joining from Atletico Madrid last summer.
Rahman is expected to play second fiddle to Spanish international Cesar Azpilicueta, but his tender age means there plenty of improvement to come from the former Greuther Furth man.
Chelsea lifted the Premier League trophy for the fourth time on Sunday following their 3-1 win over Sunderland on Sunday.
