De Bruyne has been linked with a return to the Bundesliga – where he spent last season on loan with Werder Bremen – for several weeks and Leverkusen have moved ahead of Wolfsburg and Schalke in the race for his signature.
The 22-year-old is desperate to leave Stamford Bridge after starting just two Premier League games all season and Tuesday’s Capital One Cup defeat at Sunderland is set to be his last game for the Blues.
Belgium boss Marc Wilmost has told his squad they must be playing regular first team football ahead of next summer’s World Cup in Brazil, a message that has only heightened De Bruyne’s desire to leave.
The winger was signed from Genk for £7m in January 2012, meaning Chelsea will pocket a £5.5m profit on a player who has featured just six times for the club, should the Leverkusen deal go through.
