They finished their season with a 3-1 victory over Sunderland at home, and they hoisted the trophy in front of their fans.
Manchester City, Arsenal and Manchester United respectively rounded out the top four with 79, 75 and 70 points.
Following the release of the final Premier League standings, prize money has been handed out to teams, according to their finish.
For winning the title, Chelsea has earned £24.7m in prize money. Man City, Arsenal and Man United also earned over £20m.
The rest of the top half earned over £13m in prize money, and Crystal Palace was able to land an extra £2.5m after their season-ending victory over Swansea City.
Hull City, Burnley and QPR have each been relegated from the Premier League due to finishing in the bottom three, but they will take home prize money as well.
