Cox set a career high for interceptions with five last season after returning to the 49ers. He had previously played for them from 2012-13 before being released and joining the Seattle Seahawks.
His career nearly came to an end after he was accused of sexual assault in 2010, but he was found not guilty in 2012.
Cox is expected to start at cornerback for the Titans along with Jason McCourty, who came up with 85 tackles and three interceptions last season.
NFL Media insider Ian Rapoport reported on the Titans signing Cox on Twitter.
