Baker will be making the jump to Eredivisie club Vitesse Arnhem as he looks to continue his development.
The midfielder still has yet to make an appearance for Chelsea, who took home the most recent Premier League title.
However, he was able to make the most of his season with Milton Keynes Dons, whom he had spent a season with on a loan deal.
Baker made 12 appearances for the Sky Bet League squad and scored three goals for them.
They finished second in the standings and snagged a spot in the Championship.
The young midfielder also had a brief stint with Sheffield Wednesday on a loan deal.
Meanwhile, Chelsea will look to defend their Premier League title as teams like Manchester United and Liverpool continue their aggression in the transfer window.
